300 Care & Support Services The services provided to residents of supported housing to meet their assessed needs, promote their independence, and improve their well-being. This encompasses a wide range of activities, from personal care to tenancy sustainment support.  
310 Needs Assessment The process of identifying and evaluating an individual’s needs for care and support. This can include formal statutory assessments and less formal ongoing assessments.  
310.1 Social Care Assessment A formal assessment of an individual’s social care needs, carried out by a local authority under the Care Act 2014.  
310.2 Health Needs Assessment An assessment of an individual’s health needs, carried out by healthcare professionals (e.g., NHS staff).  
310.3 Ongoing Support Needs Assessment New. Less formal, regular reviews of an individual’s support needs, carried out by support staff or other professionals, to identify any changes and ensure that the support plan remains appropriate.  
310.4 Risk Assessment (Support-Related) New. Assessment of risks related to the individual’s support needs and the delivery of support services, to inform risk management strategies within the support plan. 960
320 Care & Support Planning The process of developing an individualized plan to meet an individual’s assessed needs, outlining the specific support services to be provided, goals, and desired outcomes.  
320.1 Person-Centred Planning An approach to planning that focuses on the individual’s strengths, preferences, and aspirations, and involves them as active participants in the planning process.  
320.2 Support Plan (Document) New. The actual document that outlines the agreed-upon support plan, including details of needs, goals, services, responsibilities, and review dates.  
320.3 Crisis and Contingency Planning New. Planning for how to respond to crisis situations or unexpected events that may impact the individual’s support needs or well-being.  
320.4 Service User Involvement in Planning New. Ensuring that the individual is actively involved in the development of their support plan. 130
